If a card or creature with counters on it would be removed from the game does it lose those counters?

- When a permanent with counters on it leaves the battlefield, those counters are lost, yes. If it returns to play later, the game will consider it a new object of the same type, and it will have any counters it would normally enter the battlefield with, and nothing else.
